GP Union Threatens Labor Dispute Over Appointment Changes

General practitioners are issuing a threat to initiate a labor dispute unless Wes Streeting stops significant alterations to the method in which patients schedule appointments. The Health Secretary has pledged to eliminate the morning rush for appointments starting Wednesday by mandating English practices to enable patients to request appointments online at any time during operational hours. At present, practices can suspend online booking options during peak times, leading to engaged phone lines and hindering some patients from securing appointments.

The proposed modifications to maintain continuous online access aimed to free up phone lines for elderly individuals who rely on them. However, the British Medical Association (BMA) argues that without additional resources, it could swiftly evolve into a critical patient safety concern. The doctors’ union has indicated that they will escalate the situation and formally contest the plan if the Government does not halt it within the next 48 hours.

Dr. Katie Bramall, Chair of the BMA GP committee, expressed concerns that the changes would likely result in the establishment of waiting lists similar to those in hospitals within general practices, potentially reducing face-to-face GP consultations. This could pose risks to patients as GPs sift through a deluge of online requests to identify urgent cases amidst a significant backlog of unmet patient needs.

Wes Streeting is anticipated to address the doctors’ union during his speech at the Labour Party Conference in Liverpool tomorrow. The BMA advocates for practices to retain the capability to redirect online consultation requests to telephone or walk-in options if they become overwhelmed. However, under the current system, many individuals struggle to connect with healthcare providers.

According to the 2024 GP Patient Survey, over a quarter of individuals reported dissatisfaction with their last attempt to secure a GP appointment. The incoming Labour administration cautioned that under the Tories, GP accessibility had deteriorated to the extent that patients in certain regions had become accustomed to directly seeking care at A&E facilities.

The impending change signifies a shift where receptionists will no longer act as gatekeepers to GPs starting October 1. In England, the GP triage process will be revamped, requiring practices to accept appointment requests online, via phone, or through walk-ins from 8 am to 6:30 pm. Patients will be required to provide their condition details, following which the practice should respond within one working day, offering a GP appointment slot, a referral to a pharmacist or other NHS services, or self-care advice in some instances.

The alteration will eliminate the practice of instructing patients to call back the next day at 8 am. The BMA highlights that current online systems lack the ability to differentiate between urgent and non-urgent cases. The union stressed that their acceptance of the changes was contingent on the implementation of necessary precautions to prevent erroneous submission of urgent clinical requests online before the transition.

The BMA warns that without increased staffing capacity, GPs may need to allocate time away from face-to-face consultations to review online appointment requests. Should the change proceed, the BMA is prepared to engage in a technical dispute with the Government, potentially leading to a strike ballot if unresolved.

Dr. Bramall emphasized the importance of having necessary safeguards in place before the implementation date. The BMA asserts that while they embrace technological advancements in NHS operations, patient and staff safety must remain a top priority. The Government has been given a 48-hour ultimatum to reconsider the course of action, prevent the dispute, and uphold its commitments.
